Konstantinos Deligiannidis, MD, has been named medical director of Northwell Health House Calls, after serving as the program’s director of education and quality since 2016.
As medical director of Northwell’s House Calls, Dr. Deligiannidis provides direct care and supervises a multidisciplinary staff to deliver quality health care to vulnerable, frail elderly patients in their homes in Long Island, Queens and Manhattan.
“Dr. Deligiannidis brings strong leadership and collaboration among colleagues along with honed clinical skills in treating patients at home with multiple chronic illnesses to deliver the best possible care,” said Zenobia Brown MD, medical director and vice president of population health care management at Northwell Health Solutions. “He has proven himself to be a dedicated, value-driven and selfless physician in our field. Along with his colleagues, he stays committed to treating patients during the coronavirus pandemic and delivering Covid-19 vaccines to homebound individuals.”
Prior to joining Northwell, Dr. Deligiannidis practiced as a family medicine physician at the Barre Family Health Center at UMass Memorial Health Care in Barre, MA. He earned his BA at Harvard University, his MPH at the Boston University School of Public Health, and his MD at the University of Massachusetts Medical School.
Board-certified in family medicine, Dr. Deligiannidis holds the position of associate professor in medicine at the Donald and Barbara Zucker School of Medicine at Hofstra/Northwell. A recipient of numerous honors and awards, Dr. Deligiannidis was named House Call Educator of the Year in 2018 by the American Academy of Home Care Medicine.
Northwell’s House Calls program is a national leader in providing home-based primary care to medically complex seniors with quality outcomes recognized by the U.S. Centers for Medicare & Medicaid (CMS). Part of the federal demonstration project Independence at Home, House Calls received significant quality incentive payments in 2019 for its superior health outcomes and ability to reduce health costs at the same time.
Patients enrolled in the House Calls program receive coordinated care, similar to treatment and referrals they would receive from a primary care physician, such as ultrasounds, radiology, electrocardiogram (EKG), lab work, physical exams, occupational and speech therapy, social work services, as well as prescription refills. Physicians, nurse practitioners and other clinicians are available for urgent, same-day visits. The team is accessible 24/7 to answer clinical questions from patients and caregivers. In addition, Northwell’s Center for Emergency Medical Services works with the program to bringing highly trained community paramedics to patients’ homes to assess and treat them.
